  3. Fill out the repair form entirely with Problems/Symptoms/Fault Codes (be specific)

  5. Open the file in Acrobat Mobile Reader, fill out and save
  6. Put the Faulty Part and the Completed Repair Form in a secured box
  7. Mail your box to: Auto & Truck Electronics, Repair Department, 3600 Investment Lane, Suite 104, West Palm Beach, Florida 33404 - the address is also located at the bottom of the repair form
